Case Study: Managing Type 2 Diabetes and Hypertension with Lifestyle Changes and Ayurvedic Care

Patient Profile Mr Mahendra Deshmukh, a 51-year-old businessman from Pimpri-Chinchwad, Pune, had been living with Type 2 diabetes for three years and hypertension for five years. Along with fluctuating blood sugar levels, he experienced joint pain, swelling in his legs, occasional headaches, and persistent fatigue. For the past six months, he had also been struggling with disturbed sleep and difficulty walking properly. He runs a catering business with a demanding schedule that keeps him working from 5 a.m. to 10 p.m. The irregular meal timings that came with this routine started affecting his health. He felt older than his years, slowed by weakness and breathlessness even on mild exertion. Tired of relying on allopathic medicines and eager to feel like himself again, Mr Deshmukh wanted to lose weight, become symptom-free, and eventually stop his medication altogether. Looking for a holistic approach that addressed the root of his condition rather than just the symptoms, he chose Madhavbaug’s Morwadi, Pimpri clinic to begin his treatment programme. Initial Assessment During his first consultation at Madhavbaug, Mr Deshmukh underwent a detailed assessment of his medical history, lifestyle, dietary habits and existing health conditions to better understand the factors contributing to his diabetes, hypertension and obesity. At the time of first assessment, he weighed 92 kg with a waist circumference of 117 cm. His random blood sugar (RBS) was 156 mg/dL, HbA1c was 6.7%, and his blood pressure was 128/84 mmHg. He was taking daily medication for both blood sugar control and blood pressure management. A detailed discussion with the treating doctors revealed that his long working hours, irregular meal timings, inadequate sleep and physically demanding schedule were making it difficult for him to maintain healthy lifestyle habits consistently. These factors, together with obesity and poor metabolic health, were contributing to his overall condition. Personalised Treatment Plan Based on his assessment, Mr Deshmukh was enrolled in a three-month Comprehensive Diabetes Care (CDC 1) programme combined with a Hypertension Complication Management care plan at Madhavbaug. His treatment plan was designed around his busy schedule, with a focus on controlling his blood sugar and blood pressure, helping him lose weight safely, and easing his joint pain and swelling. The programme included: Personalised Ayurvedic medicines prescribed under medical supervision. Weekly Panchakarma-based therapies aimed at improving insulin sensitivity and supporting vascular health. A structured dietary programme using three Prameha Diet Kits over three months, based on a low-carbohydrate, high-protein, high-fibre diet. A daily exercise routine including strength training, cardio, yoga and pranayama. Regular monitoring of blood sugar, blood pressure and other vital parameters, with monthly HbA1c testing. Careful supervision while gradually discontinuing conventional medicines based on his clinical progress. Given the demands of his catering business, the programme was structured to work around his schedule, with therapy sessions and daily routines designed to be practical for him to follow consistently. Journey Towards Better Health Over the three-month programme, Mr Deshmukh remained committed to his treatment plan, attending his weekly Panchakarma sessions, following his diet kit, and completing his daily exercise routine despite his hectic work schedule. Within the first month, his blood sugar improved from 156 mg/dl to 102 mg/dl, and the swelling in his legs and breathlessness on exertion had resolved completely. By the second month, his blood sugar had stabilised further to 104 mg/dl, and his persistent joint pain had also resolved. At the end of three months, Mr Deshmukh had no remaining symptoms or complaints. His blood sugar had improved to 82 mg/dl, and his other measured results showed significant progress: his weight reduced from 92 kg to 76.2 kg, his BMI improved from 31.8 to 26.3, and his abdominal girth reduced from 117 cm to 104 cm.  His blood pressure improved to 120/80 mmHg, and his HbA1c improved from 6.7% to 5.1%, moving from the diabetic range to near-normal levels. All his allopathic medicines were gradually discontinued during this period, under the supervision of his treating doctors. At his latest follow-up, his blood sugar remained well controlled at 102 mg/dl, and he continues to report no symptoms. Improvements Beyond the Numbers Although the clinical improvements were significant, Mr Deshmukh believes the biggest change was in the way he felt every day. Before treatment, his long hours in the catering business left him with no energy, and even routine parts of his day felt harder than they should have. Following the programme, he reported feeling far more energetic through the day, even though he worked for the same hours. He now walks around 5 km daily and exercises at an open gym with ease. He has started to enjoy activities that would have been difficult for him just months earlier. He sleeps better, moves more comfortably, and is able to keep up with the demands of his business without the exhaustion that he used to feel. The weight loss also brought a renewed sense of confidence. Mr Deshmukh describes feeling physically stronger and more capable than he had in years. Patient Experience Reflecting on his journey, Mr Deshmukh expressed deep satisfaction with the outcome of his treatment. He shared that becoming free of the medicines he had depended on for years, while staying symptom-free under his doctor’s guidance, has meant the most to him. He can now climb Durga Tekadi hill with ease, something he says would have felt out of reach a few months ago, and his goal of losing 20 kg in total is close to being achieved. He credits the structured programme, regular monitoring, and a diet plan that worked around his catering schedule for helping him stay consistent throughout his treatment. Doctor’s Perspective According to the treating medical team, Mr Deshmukh’s case highlights how obesity, uncontrolled blood sugar, and poorly controlled blood pressure can worsen one another when lifestyle factors remain unaddressed. His high BMI, elevated HbA1c, and poor blood pressure control, despite regular medication, were further complicated by his joint pain, leg swelling, and breathlessness on exertion. Through a personalised programme combining Ayurvedic Panchakarma therapy, dietary correction, structured exercise, and continuous monitoring, he showed consistent improvement across every measured parameter. Every treatment plan at Madhavbaug is built around the individual patient, their
